For your convenience, we accept cash, personal checks, flexible healthcare account cards, and all major credit cards. A partial payment is expected at the time the services are performed. When more extensive dental care is necessary, financial arrangements can be made with our office, and we are pleased to offer Care Credit.
Filing claims with your PRIMARY medical/dental insurance is a courtesy that we will gladly provide within 3 days of service. Please keep in mind that the charges for any services are your financial responsibility. Full payment is due within 60 days of service regardless of insurance status. This office has no control over insurance companies and the way they expedite the processing of claims. An administrative fee of $15.00 will be assessed to your account to re-bill your insurance if incorrect information was provided at the time of the appointment.
Dr. Bergamini is not a participating member of any medical insurance plan, including Medicare. If you are part of a PPO/DMO dental plan, please confirm with your insurance company whether or not our office is a participating provider with your plan. It is your responsibility to notify this office of any changes with your insurance. We require a photocopy of your insurance card, current EOB or a completed insurance form and a driver’s license at the time of your visit.
